Indeed the issue as Giant Head is a substring of Ancient Giant Head. Renamed to Crumbled Giant Head and it should be fixed as of version 20221202 Strife 7730

I have tried both single target and group, so targeted and untargeted. Other enemies in the group ARE affected, though seem to be lower HD monsters overall. I have Extend Spell only as a metamagic feat.
