Either there are multiples overlapped or they trigger twice, having seen the screenshots they were getting hit with multiple in one round.

Quest NPC is moving outside with v8860. None of the other NPCs inside have a store.
As of v8860, the doppelganger Nora has been appropriately removed and now serves in the Last Watch legion.
