Pyre transition from "top" to "Bottom" puts you behind the transition potentially locking you in place

Issue Description: When transitioning down to the bottom level you arrive on the north end of a door that exits to the south (aka, you're behind the transition). Normally not a big deal, but fire giants block the door and can hang out on the transition if someone ran from them so you get a nice nasty surprise from which you're locked in to a murder closet where summons / companions don't follow you down because there is no room for them to jump to your location.

@Emoteen A screenshot is helpful here if you don't mind. I'd like to be able to figure out where the spawn point is that blocks the door and a visual will help since in the toolset - a spawnpoint doesn't say "fire giants here" its just a waypoint.
