Animal Companion - Death not in logs?

Issue Description:When fighting vs Treth, nothing showed in the log that my animal companion was dead, until I tried to summon it again, and I got the message 'Your animal companion has died, you must find a new one.' However, later the same day I was able to summon the same companion.
Then today, after fighting Izereth, I got the 'Your animal companion has died, you must find a new one.' message, and I am still not able to summon it. But, there is nothing in the log showing that it died. Both times I was "killed" by spells (on subdual mode) by PCs.
