Cowl of Patience quest reward no charges

Issue Description:
This Cowl dropped from one of the level 4-6 quests inside the city (sorry I don't remember which exactly as I identified it along with a batch of other quest loot after doing a few different ones) and while it has the "Use: See Invisibility (3) 1 Charge/Use" effect on it, it drops without any charges at all so its impossible to use. -
I can fix it if I know what quest it comes from. Easier than sifting through every quest area's chest :)

I did fill it out with as much info as I can. I just erased the slots I did not know instead of keeping them blank. As I said in my original post, I do not know what quest it was from because all my loot was lumped together and not identified until later when I noticed the issue. But I can refrain from posting such things in the future if I'm not 100% sure about all the details if that would be preferred.

Details help a lot or the DM team needs to spend considerable time investigating. If you can try to at least narrow it down to quest name and location that helps significantly speed up a fix/resolution.
